Patton Portraits Photo Release Form

I grant to Patton Portraits the right to take photographs of me and my family in connection with the above-identified event. I authorize Patton Portraits, its assigns and transferees the copyright, use and publish the same in print and/or electronicall

I agree that Patton Portraits may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, image competition and Web content.

I understand that without the ability to publish Patton Portraits can not distribute my images to be via webpage through image download. In which

I understand that my retainer is non refundable if I do not give more than a 24 hour notice. Sessions can be rescheduled one time by client if photographer has received 24 hour advance notice of cancellation.

*Pictures are not allowed to be taken by family member during the photography session unless it is photos of Tammy and her team working during the session.

I hereby acknowledge and agree to the following, as a condition of my photography session. 

My involvement and/or participation in my photography session is voluntary, and I am acting under my own free will.

There is a list of danger, bodily harm, injury, emotional stress or death as a result of my participation.

There is the potential for risks and dangers that may not be obvious or reasonably foreseeable at this time.

I do not have any medical aliments, physical limitations, or mental disabilities that will affect my ability to participate.

Patton Portraits undertakes no direct legal or financial responsibility for my personal safety.

I assume the risks, including, but not limited to, those outlined in Section 3 of this agreement.

I forever release Patton Portraits from any and all claims and causes of action that I or my representatives now have or may have in the future for personal injury, property damage or wrongful death occurring to me, arising out of participation in the photo session.

I am 100% liable for all medical expenses incurred as a result of any injury or property damage during my participation in my photo session.

In the event that any one or more of the provisions of this agreement shall be held to be invalid, illegal, unenforceable or in conflict with the law. The remaining portions will not be in validated and shall remain in full force and effect.

This is a legal binding contract, but is not meant to pronounce any claims or defenses that are legally prohibited.
